What Filler is Best For Me?

This is the question I hear most often in clinical practice. Patients absorb so much advertising, so many recommendations from friends, and so much conflicting information online that choosing a filler starts to feel impossible. I try to make it easy for my patients by weighing three things, in this order:

1. Safety
2. Results
3. Longevity

From there, the deciding factor is the area you are choosing to enhance — certain fillers are simply better for certain areas. What follows is the same guidance I give my own patients, drawn from years of injecting all of the major FDA-approved fillers and from the comparison studies I give the most credibility to: the ones run by independent physicians, not paid for by the companies selling the products.

The most popular FDA-approved fillers in the USA are Restylane, Juvéderm, Radiesse, and Sculptra. Here is how I think about each:

Voluma XC

Voluma is the Juvéderm filler built exclusively for long-lasting volume replacement in the cheeks and mid-face — an excellent substitute for facial fat loss, with proven longevity of up to 2 years and normal activities resumed within 24 hours. Restylane and Juvéderm

• I group these together because both are hyaluronic acid (HA) fillers, and in real-world results they are very similar despite company claims — for every patient of mine who prefers Restylane, another prefers Juvéderm. Your injector selects the specific formula per area; the full product-by-product breakdown lives on our Juvéderm & Restylane menu.
• Duration runs 6–18 months depending on the product and the area — and in my experience it builds. A first treatment metabolizes fastest; second and third treatments layer on what remains, so results hold longer with each visit.
• Uses: lips, fine lines and wrinkles, hollows under the eyes.
• The HA advantage: these are the only fillers that can be dissolved (with hyaluronidase) if you ever want to adjust or reverse a result.

Radiesse

• Composed of calcium hydroxylapatite (CaHA) — a thicker, more structural compound than the HA fillers.
• Lasts longer: patients are generally happy with the result for a year or more — roughly 10–18 months on average.
• Uses: deep folds beside the nose, marionette lines, and true volume replacement — including a remarkable effect on the back of the hands.
• In my opinion, this is the product that lets a physician augment and enhance the full face and achieve the results of a liquid facelift. Volume is the key to youthfulness.

Sculptra

• Poly-L-lactic acid (PLLA) — not a new compound; it was formulated for years in Europe as New-Fill.
• A pure volume builder rather than a line filler, so we use it much like Radiesse: to replace volume and fat loss in the face.
• The process is different: Sculptra arrives as a dry powder that we reconstitute and inject. The initial water volume fades quickly, and your body then builds collagen around the particles over the following month.
• So the rhythm with Sculptra is: 1. Inject → 2. Wait one month → 3. Re-evaluate — with volume that can last 2+ years.

Texture, Not Wrinkles?
